Description CAN Community Health is now hiring an Online PrEP Coordinator Schedule: Full-Time | Day Shift | Monday-Thursday 8:00 am - 5:00 pm Friday 8:00 am - 12:00 pm Are you passionate about patient care and ready to make a difference every day? This role promotes efficient operations, patient-centered communication, and continuity of care within the online PrEP model. The Coordinator serves as a primary operational liaison between patients and interdisciplinary teams to enhance access, retention, and satisfaction. - Upon new patient registration, verify that all required consents, demographic information, guarantor details, and required assessments are fully completed and documented in the electronic health record (EHR). - Review and assign the appropriate pharmacy in the EHR based on program protocols and regional pharmacy guidance prior to the patient’s scheduled appointment. - Conduct structured counseling for established patients requesting transfer to the Online PrEP program, clearly explaining program differences and limitations, and support informed, patient-centered decision-making to ensure alignment with individual care needs. - Perform comprehensive chart preparation for all PrEP visits, ensuring required documentation, laboratory results, and provider-directed actions are available prior to the appointment. Duties, responsibilities, and activities may change at any time with or without notice.   Requirements Education/Professional: - Bachelor’s degree in business administration, healthcare administration, public health, or a related field preferred. Relevant healthcare or patient coordination experience may be considered in lieu of a degree.  - Must be able to operate a motor vehicle and have valid insurance and driver’s license.
