Summary: Works collaboratively with physicians, staff and other health care professionals to provide care coordination across the health care continuum for all assigned patients ensuring that safety, best practice, and high quality standards of care are maintained across the continuum. Responsibilities: May assist with obtaining authorizations and scheduling where appropriate.Works closely with physicians and the multidisciplinary care management teams. Facilitates care coordination, and documents efforts.Utilizes the appropriate reports to prioritize outreach to ensure care coordination.Coordinates a wide range of activities and education to support the patient population by utilizing a variety of... communication methods including but not limited to telehealth, face-to-face, text, telephone, or e-mail.Extract data from medical record sources to gather information and perform outreach. Communicates details to the appropriate team, providers, and/or supervisors.
