About the Role: Home Health RN We're seeking an experienced and compassionate Home Health Registered Nurse (RN) to join our team at Workwarp, a part of UnitedHealth Group. As a Home Health RN, you will play a critical role in delivering high-quality patient care in the comfort of their own homes. This is a remote opportunity with immediate openings available, offering you the flexibility to work from anywhere while making a significant impact on our patients' lives. Primary Responsibilities: Organize and provide home care services, including medication administration, patient education, and wound care, among other responsibilities. Collaborate with the RN supervisor to ensure the client-driven plan of care is effective and achieves positive outcomes through ongoing evaluation using evidence-based practice. Ensure compliance with state and federal regulations by participating in chart reviews and following established processes for documentation and care delivery. Deliver efficient and valuable services that meet the expectations of quality healthcare delivery within the operational goals of Southwest Medical Home Health and UnitedHealth Group. Perform the majority of job duties in patients' homes, requiring reliable transportation and a valid driver's license. Participate in on-call rotations, work weekends, and rotating holidays as needed, with shifts that can include weekdays, weekends, or a combination of both. Take on additional responsibilities as assigned by management. Required Qualifications: Undergraduate degree in nursing or equivalent experience. Valid non-restrictive Nevada nursing license. Reliable transportation with a valid driver's license and car insurance. Proven good clinical and assessment skills, with experience in IVs, ports, drains, tubes, wound care, wound vacs, and Foley's. Willingness to travel from home to home, participate in on-call and holiday coverage rotations, and work at least one weekend day every week. Preferred Qualifications: Experience working in home health care or a similar setting. Strong knowledge of evidence-based practice and current healthcare regulations. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Skills and Competencies: Clinical expertise in home health care, including wound care and medication management. Strong assessment and critical thinking skills. Effective communication and interpersonal skills. Ability to work in a fast-paced environment with minimal supervision. Proficiency in electronic medical records and other healthcare software.
