Key Responsibilities Lead and collaborate with our care team to develop and implement comprehensive patient care plans. Provide high-quality, patient-centered care to individuals in Butler and surrounding areas. Utilize your clinical expertise to assess patient needs, develop targeted interventions, and evaluate outcomes. Work effectively in a fast-paced, dynamic environment, prioritizing tasks and managing your time efficiently. Maintain accurate and timely documentation, meeting critical deadlines for charting and other administrative tasks. Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination. Participate in ongoing education and professional development to stay current with best practices and industry advancements. Essential Qualifications To succeed in this role, you'll need: A current RN license in the state where you practice. Graduation from an approved school of practical nursing. A minimum of one year of RN experience, with demonstrated clinical competence in delivering quality patient care. A valid driver's license and the ability to spend approximately 20% of your day driving to and from patient locations. Proficiency in using tablets, mobile phones, and EMR software. Strong time management and organizational skills, with the ability to self-manage your schedule. Preferred Qualifications While not required, the following qualifications are highly valued: Experience in home health or hospice care. Knowledge of industry best practices and current healthcare trends. Certifications relevant to home health or hospice care, such as OCN or COS-C.
