Job Description:
• Evaluate and enhance the efficiency, effectiveness, and quality of work flow, and lead performance improvement efforts to identify opportunities for improvement and to implement best practice solutions.
• Manage supervisory level personnel and care coordination staff, as well as hiring and ongoing training of the care coordination team.
• Interact, facilitate, and establish partnerships and collaborations with internal and external stakeholder.
• Develop and maintain reporting mechanisms to allow for tracking, trending, and monitoring of initiatives and outcomes.
• Facilitate gathering, analyze, and present data to comply with reporting requirements.
• Ensure compliance with federal, state, and agency policies, procedures, and standards through documentation review, clinical outcomes analysis, and staff education.
Requirements:
• Registered nurse with an active license in the state of Tennessee or hold a license in the state of their residence if the state is participating in the Nurse Licensure Compact Act; OR, Masters in Social Work with an active unrestricted license.
• 5 years - Supervisory experience required
• 3 years - Experience providing care coordination to persons receiving long-term care and/or home and community based services required
• Analytical and decision making abilities.
• Proficient in Microsoft Office (Outlook, Word, Excel and PowerPoint)
• Excellent oral and written communication skills
• Good interpersonal skills and teamwork ethic
• Be self-motivated and capable of working independently
• Be organized, reliable, and able to manage multiple tasks with exceptional work ethic.
• Ability to lead, train, motivate, delegate, follow-up, resolve and overcome obstacles to meet aggressive time tables.
