Job Overview: We're seeking a compassionate and experienced Registered Nurse to join our team as a Case Manager, working remotely from North Carolina. As a key member of our Aetna One Advocacy team, you'll play a vital role in delivering personalized, high-touch care to our members, empowering them to take control of their health and wellbeing. About the Role: In this dynamic and rewarding position, you'll utilize your clinical expertise to assess, plan, implement, and coordinate comprehensive care plans for our members. Through telephonic and face-to-face interactions, you'll evaluate medical needs, facilitate wellness initiatives, and provide ongoing support to enhance short-term and long-term outcomes. Your expertise will be instrumental in reducing risk factors, addressing complex health and social indicators, and promoting overall wellness.
