Position Overview:
As a Remote Primary Nurse, you will deliver compassionate, comprehensive, and evidence-based care to patients managing chronic conditions. You’ll collaborate with a multidisciplinary team, utilize cutting-edge telehealth technology, and empower patients to achieve better health outcomes.
Key Responsibilities:
- Serve as the primary point of contact for chronic care patients, offering support, education, and personalized care plans.
- Conduct telephonic or video assessments to evaluate patient needs, progress, and adherence to treatment plans.
- Coordinate care with healthcare providers, specialists, and community resources.
- Monitor and adjust care plans based on patient outcomes, ensuring optimal management of chronic conditions.
- Provide education on disease management, medication adherence, and lifestyle modifications.
- Document all patient interactions and maintain compliance with clinical protocols and HIPAA regulations.
Qualifications:
- Active and unrestricted Registered Nurse (RN) license.
- Minimum of 3 years of clinical experience, preferably in chronic care, case management, or telehealth.
- Strong communication and interpersonal skills, with a passion for patient advocacy.
- Familiarity with electronic medical records (EMR) systems.
- Ability to work independently in a remote setting while collaborating effectively with a team.
Preferred Skills:
- Experience in telehealth or remote nursing roles.
- Certification in chronic care or case management (e.g., CCM, CMCN).
- Bilingual skills are a plus.
