You will: Coordinate patient care across the continuum of care, including referrals, home visits, and community resources. Assess patient needs and develop care plans to improve health outcomes and ensure access to benefits. Communicate with patients and partners to provide education, support, and advocacy. Maintain accurate and timely documentation of case management activities. Collaborate with a multidisciplinary team to optimize patient well-being.
