Remote Medical Assistant/LPN for Virtual Patient Care (Bilingual EN/SP Preferred)
Part-Time, Full-Time | Flexible Hours | US-Based Only | Contract/1099
Lara Health is seeking a compassionate and experienced Medical Assistant (CMA) / Licensed Practical Nurse (LPN/LVN) to join our Chronic Care Management (CCM) / Remote Patient Monitoring (RPM) team. This is a fully remote role with flexible scheduling, ideal for someone passionate about chronic care and patient engagement.
Position Highlights
• Schedule: Minimum 25 hours/week
• Location: Fully remote (U.S. based only)
• Experience Required: Minimum 5 years caring for patients with chronic conditions (e.g., diabetes and HTN, cardiology, primary care)
Role Responsibilities
• Monitor patient vitals (blood pressure, blood sugar, etc.) and medication adherence
• Support patients in scheduling MD visits and making lifestyle changes
• Maintain accurate and up-to-date EMRs
• Collaborate with providers and care team
• Perform other duties as assigned
Required Skills & Qualifications
• Licensure: Active MA/LPN license (mandatory)
• Experience: 5+ years in clinical setting, preferably in chronic care settings
• Language: Bilingual English/Spanish strongly preferred
• Strong communication, organizational, and time management skills
• Ability to work independently and collaboratively
• Proficiency with computers and EMR systems (must have own device and reliable internet)
• Familiarity with medical terminology and clerical tasks
• Passion for working with older adults and delivering preventative care
Preferred Experience
• Familiarity with remote care coordination tools and EMR-based task management
• Ability to delegate, prioritize, and oversee multiple patient cases simultaneously
