Registered Nurse (RN) Care Manager (Geriatric Population) Highlights Monday–Friday, 8:00 AM–5:00 PM EST Field role, with 80% travel required within a 2.5–3-hour radius from home for in-person member visits. Must be based in or near Rocky Mount, NC Registered Nurse (RN) Care Manager (Geriatric Population) Responsibilities Manage a caseload of members with long-term care needs. Conduct assessments with members, caregivers, and providers to determine support systems and care needs. Monitor service delivery and follow up via in-person visits and phone contact. Authorize and coordinate service referrals. Identify deficiencies in care plans and ensure seamless service delivery. Collaborate with discharge planners, physicians, and community partners. Help members file and resolve complaints and appeals. Document visits and maintain records using electronic medical health records. Registered Nurse (RN) Care Manager (Geriatric Population) Requirements RN or LCSW license required. Bachelor’s degree required. 2+ years of experience in care management (4–6+ years preferred) Experience in physical health, utilization management, long-term care, or home health Proficiency in Microsoft Office and EMR software Must have a valid driver’s license. Attendance is mandatory for the first 90 days.
