This remote role focuses on telehealth care coordination and virtual nursing assessments to support underserved populations with chronic condition management. You will join a multidisciplinary team focused on value-based care, offering personalized nursing care plans, smooth care transitions, and patient educationwithout in-home visits, weekends, on-call shifts, or holiday work.
