Job Summary
Under general supervision, the Patient Record Abstractor III performs advanced-level abstraction, analysis, and review of patient medical records to support hospital data processing, reporting, and documentation compliance. This role involves complex record analysis, documentation review, and coding-related abstraction functions at an advanced operational level.
The position requires strong experience in Radiology, Emergency Medicine, and Ophthalmology documentation review and abstraction.
Key Responsibilities
• Review, analyze, and abstract patient medical records for accuracy and completeness.
• Perform complex and detailed coding and abstraction of patient records for data processing and analysis.
• Identify and extract relevant clinical and diagnostic information from medical documentation.
• Ensure compliance with organizational policies and healthcare documentation standards.
• Conduct comparative analyses of patient records and prepare summaries as required.
• Recognize and resolve discrepancies in medical documentation.
• Support data retrieval, reporting, and patient record organization.
• Maintain patient record indices and documentation systems.
• May provide supervisory support or technical guidance to lower-level abstractors, if assigned.
• Perform other duties as assigned.
Required Qualifications
• High school diploma or General Education Diploma (GED).
• Demonstrated knowledge of medical terminology.
• Minimum three (3) years of related hospital clerical experience.
• At least two (2) years of experience in patient record abstraction or similar work.
• Strong experience with:
• Radiology documentation
• Emergency Medicine (EM)
• Ophthalmology records
• Ability to analyze complex medical documentation.
• Strong attention to detail and organizational skills.
• Ability to work independently in a remote environment.
Preferred Qualifications
• Experience in hospital medical records or health information management departments.
• Experience performing complex patient record abstraction and analysis.
• Prior supervisory or technical lead experience.
