Location: Indianapolis area Bilingual: a plus not required Hours: Part time & Full Time Company Culture: Led by a Nurse Practitioner Values Nurse Practitioners Environment: Less stressful, allowing time for high-quality education and care Why Transtreme: CEO/Founder: Practicing NP who understands your role Mission: Address patient care concerns, burnout, lack of quality care, and escalating costs Team: NPs across multiple states, collaborating with Medical Provider Groups, Health Plans, and serving hundreds of thousands of patients Job Duties: Patient Care: Work with geriatric patients Complete Medicare annual health assessments during home visits Patient Load: 5-6 patients/day This role includes seeing patients in home visits Clinical Responsibilities: Perform H&Ps Use preventative health screening tools (PAD and Neuropathy, diabetic retinal cameras, A1c and kidney testing for diabetics) Navigate and chart via EMR systems This position focuses on completing comprehensive health assessments and collaborating with patients' primary care providers to ensure continuity of care. You will enhance patient outcomes through detailed evaluations and effective communication, but this is not a primary care or PCP role. Training: We provide comprehensive training to ensure you feel confident and prepared in your role. Our supportive environment means you won't be left to navigate challenges alone.
