Responsibilities: Perform patient assessments and collaborate with the care team to develop and implement a plan of care Promote patient health and independence through teaching and appropriate rehabilitative measures Supervise LPNs and HHAs, complete documentation timely and accurately, and regularly communicate patient progress to the clinical manager and care team Participate in clinical development and continuing education programs, and perform on-call responsibilities and on-call services to patients/families as assigned Qualifications: One (1+) year of clinical experience as a Registered Nurse (RN) Current RN license, specific to the state(s) you are assigned to work Current CPR certification, valid driver's license, reliable transportation, and liability insurance How to Apply: If you are a motivated and compassionate individual ready to contribute to a thriving team, we encourage you to apply now!
