As a Field Case Manager RN, you'll play a critical role in assessing, planning, and implementing care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care. You'll also identify, initiate, and follow up on referrals for community-based programs and providers, manage the care plan throughout the continuum of care as a single point of contact, and communicate with all stakeholders to ensure quality coordinated care and services are provided expeditiously to all members. Primary Responsibilities As a Field Case Manager RN, your primary responsibilities will include: Assessing patients' needs and developing individualized care plans that address their physical, mental, and social needs Identifying and initiating referrals for community-based programs and providers, including financial, psychosocial, community, and state supportive services Managing the care plan throughout the continuum of care as a single point of contact, ensuring seamless transitions and coordination of care Communicating with all stakeholders, including patients, families, healthcare providers, and community-based organizations, to ensure quality coordinated care and services are provided expeditiously to all members Advocating for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team Requirements To be successful in this role, you'll need to possess the following qualifications and skills: Essential Qualifications: Current, unrestricted RN license in Washington DC 3 years of experience in a hospital, acute care, or direct care setting Demonstrated ability to type and navigate a Windows-based environment Access to reliable transportation Ability to work in a field-based capacity in Washington, DC Preferred Qualifications: BSN Certified Case Manager (CCM) A background in managed care Case management experience Experience or exposure to discharge planning Field-based experience going into homes Skills and Competencies To excel in this role, you'll need to possess the following skills and competencies: Strong assessment and planning skills, with the ability to develop individualized care plans that address patients' physical, mental, and social needs Excellent communication and interpersonal skills, with the ability to communicate effectively with patients, families, healthcare providers, and community-based organizations Strong problem-solving and critical thinking skills, with the ability to identify and resolve complex care coordination issues Ability to work in a fast-paced, dynamic environment, with multiple priorities and deadlines Strong computer and software navigation skills, with the ability to navigate electronic health records and other software applications Career Growth Opportunities and Learning Benefits At Optum Home & Community Care, we're committed to helping our employees grow and develop their careers.
