**Job Summary**
We're seeking an experienced Licensed Social Worker to join our remote team. As a Licensed Social Worker, you'll work collaboratively with registered nurse case managers to assess, plan, and advocate for members with financial, transportation, safety, and housing needs. This role requires an LSW/MSW with an unrestricted active license and 2+ years of experience in medical social work or case management.
**Responsibilities**
* Research available options and resources for members with financial, transportation, safety, and housing needs
* Collaborate with registered nurse case managers to assess, plan, and advocate for members
* Empower members to make positive lifestyle and medical choices
* Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
* Utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
* Conduct telephonic or electronic exchanges with members to assess their needs and provide support
**Requirements**
* Active and unrestricted LSW - Licensed Social Worker, LMSW - Licensed Masters Social Worker, LCSW - Licensed Clinical Social Worker, or LISW - Licensed Independent Social Worker in state of residence
* 2+ years of experience in medical social work or case management
* Masters Degree in Social Work
**Pay and Benefits**
* The typical pay range for this role is: $46,597.20 - $101,228.40
* This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls
* The actual base salary offer will depend on a variety of factors including experience, education, geography, and other relevant factors
* This position is eligible for a arenaflex bonus, commission, or short-term incentive program in addition to the base pay range listed above
