Key Responsibilities Perform comprehensive patient assessments and collaborate with the care team to develop and implement personalized plans of care. Make referrals to other disciplines as necessary, ensuring patients receive the support they need. Promote patient health and independence through education and rehabilitative measures, empowering patients to manage their care effectively. Supervise Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs), providing guidance and oversight as needed. Maintain accurate and timely documentation, ensuring seamless communication across the care team. Regularly update the clinical manager and care team on patient progress, facilitating coordinated care. Participate in staff education and training, contributing to the ongoing development of the team. Perform on-call responsibilities, providing support to patients and families as needed. Engage in clinical development and continuing education programs, staying up-to-date with best practices in home care. Qualifications and Requirements To succeed as a Home Care RN with Amedisys, you'll need: A minimum of one year of clinical experience as a Registered Nurse (RN), with a current RN license in the state(s) where you'll be working. Current CPR certification, ensuring you're equipped to respond in emergency situations. A valid driver's license, reliable transportation, and liability insurance, as you'll be required to travel to patient homes. If you have less than six months of clinical experience, you'll be required to participate in our RN Intern program.
