Case Manager (Remote) - Skilled Nursing Facility
Overview: We are seeking a motivated and organized Case Manager to join our skilled nursing facility team in a remote capacity. As a Case Manager, you will play a crucial role in ensuring the continuity of care by managing continued stay requests and insurance updates with insurance companies. Responsibilities:
• Manage and submit continued stay requests to insurance companies in a timely manner.
• Update insurance companies on patient progress and treatment plans as required.
• Review and document insurance coverage and reimbursement details accurately.
• Collaborate with healthcare providers to gather necessary clinical information for insurance submissions.
• Monitor and track insurance authorization timelines and requirements.
• Maintain comprehensive and organized records of insurance communications and patient information.
• Utilize electronic health record (EHR) systems proficiently to input and retrieve patient data.
• Ensure compliance with regulatory requirements and standards related to insurance authorization processes.
• Communicate effectively with internal team members and external stakeholders to facilitate smooth care transitions.
Requirements:
• Proven experience as a Case Manager in a healthcare setting, within a skilled nursing facility or similar environment.
• Experience in doing SNF authorizations and continued stay requests
• Experience in doing Part B therapy authorizations
• Strong understanding of insurance authorization processes and requirements. Understanding of Medicare, Medicaid, Insurance payer sources.
• Proficiency in using computer systems and EHR software; comfortable with technology and adapting to new platforms.
• Ability to work independently and remotely while maintaining high productivity and attention to detail.
• Effective communication skills, both written and verbal.
• Commitment to maintaining patient confidentiality and adhering to ethical standards in healthcare.
